There are three sections in the research-based disclosure form:

  1. Introduction and Acknowledgement
  2. Active Investigator determination
  3. Financial Interests and External Entities

 

On the research-based disclosure landing page, the first section, Introduction and Acknowledgement, is available for you to complete:

You are asked to review relevant information, then respond to the Acknowledgement statements. A response is required in order to proceed to the next section.

To enter the next section, you must scroll to the top of the page and either click the “Next” button (top right) or, click the next Section in the left-side column:

Note that sometimes there is a considerable lag before the form responds and reveals changes or updates based on your responses. 

 

The second section, Active Investigator Determination, is similar – you are asked to review relevant information and answer 1 or 2 questions to determine your Investigator status.

Once completed, the third section, Financial Interests and External Entities becomes available; or, you might be finished with your disclosure, if there are no more questions or sections to complete.

You are finished when the following section appears, and the “Submit” button becomes available to you:

The "Submit" button is located underneath your name, at the top of the page.

 

The third section, “Financial Interests and External Entities” begins with a screening question, based mostly on the types of external financial interests that are covered by the related policy and federal regulation.

Once completed, there might be additional screening questions that become available, depending on your response(s) to the initial question. 

Note that you can (and should) save your responses by clicking the “Save” button at the top of the screen, under your name.

 

If there is an external interest that requires additional information for review, a subsection will become available. Click the “+ Add New Relationship” button in the lower right corner to begin:

The first question, “Select Organization”, is required, and is easy to miss.  This is where the name of the external entity (organization, company, etc.) associated with your external interest, will populate once you choose it.  Start by clicking the "Find external organization" link:

The External Org Finder window will open, and you can search for the entity name in the available list:

If you cannot find the entity in the available list, then click “+ Request New External Org", located in the bottom right corner of the window:

Enter the name of the entity as accurately as possible and click “Save”:

Once the name is saved, review the initial “Select Organization” question to ensure that the entity name is entered as your response:

If entity name did not populate underneath "Select Organization", then click “Find external organization” to go back to the External Org Finder window. Search for the entity in the list, select it, and click “Save”, as shown below:

Complete the remaining subsection questions as accurately and completely as possible.

Use a new subsection for each separate entity or interest that applies, according to the disclosure form guidance. New subsections can be added to a disclosure by clicking "+ New Relationship".

All the information you enter into subsection fields will be saved, then copied into your next new research-based disclosure in Cayuse Platform.  You will be able to review, edit as necessary, and submit – without the need to type the information in again.

 

When there are no more questions or sections to complete, the “Submit” button will become available to you.  The “Submit” button is located underneath your name, at the top of the screen. 

If you want to complete the form but the “Submit” button is greyed out and not available, then there is one or more required questions that need to be answered.  If you completed a subsection, review it to make sure that your entity name is entered in the first question (“Select Organization”), then review for any other missed questions.
