The animal forms below can now be found on iRIS and orders are made online. Training is available on how to use the iRIS system.
- Animal Order Form. To order or receive laboratory animals from an approved commercial vendor.
- Animal Transfer Form. If you would like to transfer animals from one ACUP # to another ACUP # or transfer to one scientist to another scientist at Oregon State University.
