Finding the COI Disclosure Form in Cayuse
Log in to OSU's Cayuse Platform using your ONID credentials.
- Click on your name in the upper right corner, then click My Profile in the dropdown list.
- In the left-side column, scroll down to the bottom and click COI Disclosures.
- Click the + New Disclosures button. Always choose + New Disclosures instead of modify.
- Choose Start a Research-based Disclosure (note that the Annual Disclosure option is not in use at this time) then click Next.
- Feel free to click No thanks if the Cayuse Help Center pop-up comes up (and see the screenshot instructions for more information about how to access Cayuse Help)
- You should land on the first section of the research-based disclosure form and can begin.
Log into OSU's Cayuse Platform using your ONID credentials.
This is different than the login and website currently used for Cayuse sponsored programs (SP & 424) proposals.
The Cayuse Platform login link will route you through OSU's central authentication/ single sign-on system prior to entering OSU’s Cayuse Platform website.
The Cayuse Platform home page is also your My Tasks page, and looks like the screenshot below.

Click on your name in the upper right corner to reveal a dropdown list. From the dropdown list, click My Profile.

On the My Profile page, there is a column on the left side of the page. Click COI Disclosures, at the bottom of the column.

You will arrive at the following screen. From here, click New Disclosure.

You will see a pop-up screen which prompts you to choose a type of disclosure. Choose Research-based Disclosure then click Next.

At this time, you may get a helper pop-up window as shown below. Click No Thanks.
You can get the same information by clicking on the orange circle with a question mark in the lower right corner of each disclosure screen once you begin the form.

You should then land on the screen below. This is the start of your Research-Based Disclosure form.

Note that you can directly access Cayuse’s Help Center by clicking the orange circle around the question mark, in the lower right corner.

Completing the COI Disclosure Form
There are three sections in the research-based disclosure form:
- Introduction and Acknowledgment
- Active Investigator Determination
- Financial Interests and External Entities
Section 1
On the disclosure landing page, the first section, Introduction and Acknowledgment, is available for you to complete. You are asked to review relevant information, then respond to the Acknowledgment, which is required.
Once the first section is completed, the second section becomes available to you. To proceed, you must scroll to the top of the page and either click the Next button on the right. Or, click the second section within the left-side column.
Note that sometimes there is a considerable lag before the form responds and reveals changes or updates based on your responses.
Section 2
The Active Investigator Determination section is similar to the first section – you are asked to review relevant information and answer one or two questions.
Once completed, either the third and final section becomes available to you; or, you are finished with the form and can click Submit at the top of the page.
Section 3
The third section, Financial Interests and External Entities, begins with a screening question about external financial interests that are covered by the related policy and federal regulation. Additional screening questions may then become available, depending on your response(s) to the initial question.
Note that you can (and should) save your responses periodically by clicking Save at the top of the screen, as you complete the rest of your form.
If you have an external financial interest that requires additional information for review, a subsection will become available that contains additional questions about that interest type.
Click the + Add New Relationship button in the lower right corner to open a new relationship subsection.
- The first subsection question, Select Organization, is required but easy to miss. This is where you will enter the name of the external entity (organization, company, etc.).
- Click the Find external organization link to open the External Org Finder window, and search for the entity name. Select it from the list and click Save.
- If you cannot find the entity within the available list, then click the + Request New External Org button, located in the bottom right corner of the screen. A new popup window will appear with a prompt to enter the entity name. Please enter the name as accurately as possible, and click Save.
- Review your Select Organization response, and ensure that the entity name is entered as your response.
Complete the additional subsection questions as accurately and completely as possible.
Note all the information you enter in subsection fields will be saved and copied over to your next new research-based disclosure in Cayuse Platform. You will be able to review, edit as necessary, and submit – without the need to type the information in again.
When there are no more questions to complete, the Submit button (located at the top of the page) will become available to you. Click Submit when you are finished and ready to submit your form.
If you are finished with the form but the Submit button is not available, then there is a required question within the form that still requires a response. If you completed one or more subsections, review them to make sure that your entity is entered in the first question (Select Organization), then review for any other missed questions.
There are three sections in the research-based disclosure form.
- Introduction and Acknowledgment Active
- Investigator Determination
- Financial Interests and External Entities
On the research-based disclosure landing page, the first section, Introduction and Acknowledgment, is available for you to complete.

You are asked to review relevant information, then respond to the Acknowledgment statements. A response is required in order to proceed to the next section.

The second section, Active Investigator Determination, is similar – you are asked to review relevant information and answer 1 or 2 questions to determine your Investigator status.

Once completed, the third section, Financial Interests and External Entities becomes available. Or, you might be finished with your disclosure, if there are no more questions or sections to complete.
You are finished when the following section appears, and the Submit button becomes available to you.

The Submit button is located underneath your name, at the top of the page.
The third section, Financial Interests and External Entities begins with a screening question, based mostly on the types of external financial interests that are covered by the related policy and federal regulation.

Once completed, there might be additional screening questions that become available, depending on your response(s) to the initial question.
Note that you can (and should) save your responses by clicking the Save button at the top of the screen, under your name.
If there is an external interest that requires additional information for review, a subsection will become available. Click the + Add New Relationship button in the lower right corner to begin.

The first question, Select Organization, is required, and is easy to miss. This is where the name of the external entity (organization, company, etc.) associated with your external interest, will populate once you choose it. Start by clicking the Find external organization link.

The External Org Finder window will open, and you can search for the entity name in the available list.

If you cannot find the entity in the available list, then click + Request New External Org, located in the bottom right corner of the window.

Enter the name of the entity as accurately as possible and click Save.

Once the name is saved, review the initial Select Organization question to ensure that the entity name is entered as your response.

If entity name did not populate underneath Select Organization, then click Find external organization to go back to the External Org Finder window. Search for the entity in the list, select it, and click Save, as shown below.

Complete the remaining subsection questions as accurately and completely as possible. Use a new subsection for each separate entity or interest that applies, according to the disclosure form guidance. New subsections can be added to a disclosure by clicking + New Relationship.
All the information you enter into subsection fields will be saved, then copied into your next new research-based disclosure in Cayuse Platform. You will be able to review, edit as necessary, and submit – without the need to type the information in again.
When there are no more questions or sections to complete, the Submit button will become available to you. The Submit button is located underneath your name, at the top of the screen.
If you want to complete the form but the Submit button is grayed out and not available, then there is one or more required questions that need to be answered. If you completed a subsection, review it to make sure that your entity name is entered in the first question (Select Organization), then review for any other missed questions.
